Cancer Care Coordinator

A cancer care coordinator is a health professional who coordinates patient care, liaises with the range of health professionals involved in the patient treatment, and supports them and their family throughout their cancer experience. They provide extra time, capacity, and expertise to support patients in preparing for clinical conversations or in following up discussions with primary care professionals. They work closely with GPs and other colleagues within the primary care network (PCN) to identify and manage a caseload of patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed.

Learning Disability Coordinator

People with a learning disability have worse health than people without a learning disability and are more likely to experience a number of health conditions.  Eston PCN have recruited a learning disability care coordinator who delivers effective, coordinated care for people with learning disabilities their families and carers.   They are dedicated to providing bespoke and high-quality care to people in different care settings.

They are skilled in supporting people with a range of neurodevelopmental differences, neurological differences, or complex needs, including:

  • Learning disabilities
  • Autism
  • Mental health needs
  • Physical disabilities

Support workers are compassionate and empathetic individuals who aim to ensure the health and well-being needs of the patients they support, enabling them to achieve personal goals and aspirations.

Alcohol Care Coordinator

The alcohol care coordinator helps patients who are looking for support. Patients will find a warm welcome, non-judgmental support and advice.  If you feel as though you are drinking more than you would like, then our alcohol care coordinator is here to support you. They listen and never judge.  They understand that everyone’s needs and circumstances are different. They will work with you on your own goals, whether that’s cutting down your drinking, stopping completely or getting some helpful advice.  They have a range of options to support with your recovery.
