Our Primary Care Network is made up of four member practices, who collaborate to provide services more effectively to their local population.

To achieve the best outcomes for patients, we are working in partnership with other local providers and community groups to ensure our services reflect both local health needs and are personalised to individual patients and residents. Our Team

The Eston PCN Board consists of a PCN Manager with clinical and managerial representatives from each of the four member practices. It is led by a GP who is a nominated Clinical Director Dr I.A. Lone

They are supported by a management team consisting of PCN Manager Kendra Green, Digital Transformation Lead Sudhir Jayakrishna

The PCN Board, Management Team, and GP Clinical Leads work to drive collaboration between the practices and external stakeholders to improve health outcomes and increase the clinical time that the Primary Care Team can provide to patients in Eston PCN. The desired outcomes being improved health for patients and better accessibility to Primary Care services when needed. Team works very closely with Practice Managers of all the four practices.
